K-DOCK Radio and Papa Murphy’s Pizza would like to recognize local Teachers who have made a difference in the lives of their students during the week of May 7th.

Now through May 6th, in 250 words or less describe why your teacher deserves our recognition. You could win a pizza for the entire class, a gift for your teacher plus we’ll recognize the winner and runner’s up during National Teacher Appreciation Week.

• Must be a current teacher for one of the following districts (Coos Bay, North Bend, Reedsport, Bandon, Coquille, Myrtle Point, Powers)
• Must be a student enrolled in any of the following districts (Coos Bay, North Bend, Reedsport, Bandon, Coquille, Myrtle Point, Powers)
• Nominee grants K-DOCK Radio the permission to use the name and photo on the air and digitally.
• Nomination must include: 1) Name of the person making the nomination, contact phone number, email address and a nomination essay (250 words or less) 2) Teachers Name, grade taught, school, contact number or e-mail.
• All nominations submitted should be considered property of KDCQ FM and Bay Cities Building Company, Inc and will be used to promote teachers making a difference during the week.
• K-DOCK Agrees to notify the School Officials and Board of Directors of the winning nominations.
