OR Emergency Management Wildfire Disaster Survey

The Governor’s office, working with Oregon Emergency Management, is seeking information to determine the need to seek a Small Business Administration economic disaster declaration as a result of the wildfires in southern Oregon. To determine if there is a need for this declaration, they are requesting potentially affected businesses to fill out a short one page survey, which can be found at www.dcso.com/documents/sba_eidl_worksheet_questionnaire.pdf. The survey does not commit a business in any way.

The specific SBA program, if made available, would be the Economic Injury Disaster Loan program, that provides low interest loans for “working capital” intended to help the affected businesses get through the period of low revenue. Completing the form neither commits the small business to later apply for a loan, nor does it commit the SBA to make a loan. All the questionnaire does is make the case for economic injury so that the Governor can make a request for the program.

These are form fillable PDFs and can be filled out on line, but must be printed and either sent via mail or scanned and emailed. The questioners must be returned no later than August 21st, 2013.
